Community Living Support Staff

Albuquerque and Valencia County

General Description of Duties

Assist clients in their family homes or independent living environments in acquiring skills and/or completing activities of daily living. May include assistance with: banking, budgeting, paying bills, shopping, community safety skills, home management, cleaning, cooking and community recreation and leisure activity identification and participation. Will work flexible hours including evenings and weekends as necessary to meet client needs and schedules.

Education and Training

High School Diploma (or equivalent) required. Years of similar experience may be substituted for education in some instances. Experience working with MR/DD population is a plus.

Other Requirements

Must have related experience and be able to work independently, without a great deal of supervision. Candidates will use their own vehicle, so you must have reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance. Must pass TB test and background check (Mandatory finger printing and screening by State of NM and FBI). Must have effective verbal communication and writing skills. Must be able to lift 50 lbs repeatedly throughout the shift as well as support 70-100 lbs. occasionally. Adelante is a drug-free workplace.

Health Care Support Staff

Full-time position, Community Living, Albuquerque

General Description of Duties

Under supervision of the Director of Nursing, will be responsible for ensuring assigned health care services are provided/met in a timely manner. Will perform Community Living Support Staff duties as required. Responsible for ordering, picking up, and delivering medication and supplies to assigned residential homes. Will accompany clients to community based health care providers and may be responsible for accompanying clients who require emergency or urgent care to appropriate providers. Will ensure all follow-up directives are relayed to all relevant parties and perform all related administrative duties. Will staff residential homes when consumers are unable to attend day programs, school, or work activities, as well as assists clients with all personal care. Must interact positively and constructively with all parties including: Guardians, Case Managers, Therapists, Health Care professionals, all agency personnel, etc. Will attend all trainings and other related meetings as required. Will perform other duties as assigned.   

Education and Experience

High school diploma or equivalent required. Five (5) years direct experience with developmentally disabled population including personal care and health care issues required. Must maintain CPR, First Aid and other trainings/certifications as required.

Other Requirements

Must pass TB test, background check, and drug screening. Must have own reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance. Must be able to lift 25 - 50 lbs occasionally. Must be sufficiently mobile to travel to various locations without prior notice. Adelante is a drug-free workplace.

 

Independent Living Support Staff

Albuquerque; Must be available evenings and weekends..

Intensive Skills Trainer

Full-time position

Description of Duties

Responsible for assisting adults with special needs in a day program setting for a work-like environment and/or training of daily living skills. This may include assistance with eating, as well as all aspects of personal care. May include working with individuals with severe behavior problems. Will accompany residents on community outings. Must be able to document individual’s progress and make recommendations for program changes.
Education & Training

High School Diploma (or equivalent) preferred with post high school, college preferred. Experience working with mental retardation and developmental disabilities preferred. Must complete specific certifications (from our training department) and maintain those certifications throughout employment.

Must pass TB test and background check (Mandatory finger printing and screening by State of NM and FBI) and drug screening. Must have own reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance. Must have effective verbal communication and writing skills. Occupational Therapist

Exempt position, must be able to work various hours as necessary to meet client needs

General Description of Duties

Under indirect supervision, provides Occupational Therapy to improve and/or maintain fine motor skills, coordination, sensory integration and facilitating the use of adaptive equipment or other technology for individuals with mental and physical disabilities or functional limitations. Develops and implements occupational therapy programs to assist clients regain physical or mental functioning to adjust to their disabilities; tests and evaluates clients for therapy and support needs, availability of facilities, equipment and services; refers clients to appropriate specialists to design, fabricate and fit splints, braces and other selective orthotic or prosthetic adaptive devices and trains clients on use according to therapy program; monitors client progress throughout treatment; ensures adherence to procedural safeguards and confidentiality requirements. Develops perceptual motor skills, play, pre-vocational, leisure capacities and sensory integrative functioning; uses specifically designed crafts and exercise to enhance functional performance that adapts to the client’s individual environment; evaluates, designs, and implements programs to meet the individual’s needs in the areas of seating, positioning, feeding; observes, assesses and records symptoms, reactions and progress of clients. Coordinates treatment planning with other disciplines within agency and those involved in the community; effectively educates, counsels and communicates test results, diagnoses and/or treatment in a manner easily understood by clients, families and staff; instructs clients and/or families on home programs; participates in training of all relevant staff and nursing personnel in occupational therapy techniques and objects. Participates in various meetings, Interdisciplinary Team, own performance review; provides input on try-outs and peer reviews as required.

Education and Experience

Candidate must be a graduate of an accredited college or university with a Bachelor's, Master's, or PhD in Occupational Therapy. One (1) year experience in physical dysfunction settings with developmentally disabled individuals preferred. You must have current licensure by the New Mexico Occupational Therapy Board in addition to certification in first aid, CPR, Handle with Care techniques and other appropriate intervention techniques as required.

Other Requirements

Must pass TB test, background/fingerprint investigation and pre-employment drug screening. This person must function independently and have flexibility and personal integrity with clients, personnel and other support agencies. The ability to communicate effectively, clearly and legibly (written and verbal) is required. Ability to accurately record and maintain detailed information. Skill in basic computer system operation is required. Must be able to walk, stoop, bend, stand, sit, push, pull, and lift intermittently throughout the day. Must be able to lift 50+ lbs. regularly using lifting aids. Must have own, reliable transportation and an insurable driving record. Physical Therapist

Exempt position, hours vary to meet client needs

General Description of Duties

Under indirect Supervision, provides physical therapy to maintain gross/fine motor skills, facilitate independent functioning, and prevent progressive disability to individuals with physical, mental or functional limitations in direct serve areas and other agency programs. Develops and implements individual programs to assess the client’s specific needs in fine/gross motor skills, in coordination with family and other team members; develops therapy plan with other IDT members, provides strategies and refers family members to available community resources; recommends therapeutic services (i.e., methods, frequency, intensity of services needed, strategies to achieve ISP outcomes, anticipated duration of need for services, etc.); ensures adherence to procedural safeguards and confidentiality requirements. Utilizes physical or chemical properties (i.e., heat, cold, light, sound, water, air, electrical, electromagnetic, massage, exercise, mechanical application, etc.) as treatment; provides consultation and/or training to staff that includes assists in arranging the physical environment, preparing materials for activities, defining the intended purpose and recommended strategies, modeling appropriate positioning, handling, and facilitation techniques, assisting with transitions between activities and the overall implementation of the therapy plan. Designs, fabricates or modifies assistive technology or adaptive devices; provides assistive technology services; assists staff and family to obtain equipment related to motor needs. Coordinates treatment planning with other disciplines within agency and those involved in the community; effectively educates, counsels and communicates test results, diagnoses and/or treatment in a manner easily understood by clients, families and staff; instructs clients and/or families on home programs; participates in training of all relevant staff and nursing personnel in physical therapy techniques and objects. Residential Program Manager

Exempt position, hours vary to meet client needs

General Description of Duties

Responsible for the overall operation, administrative duties and supervision of staff at a single residential home. Requires shift work and on-call responsibilities. Required to carry a pager/cell phone at all times in order to be the first responder to emergencies that may arise at assigned home. Must be self-directed. Learns and utilizes resources with guidance. Implements policies, procedures and training programs to develop or increase skill/productivity in vocational and/or daily living tasks of individual clients; participates in the ISP process; assists in the admission of new clients/associate employees to assigned residence. Ensures staff training requirements are current; ensures staff scheduled for required trainings; maintains positive working relationships and professional level contacts with individuals or entities. Attends/conducts various meetings as required. Interviews new candidates and makes recommendations; conducts performance reviews, recommends disciplinary actions for assigned staff; performs other duties as assigned.

Education and Experience

Must be 21 years of age. High School Diploma or GED required. One (1) year experience working directly with DD adults in a residential environment required. Associates degree in related human services field preferred. One year supervisory experience strongly preferred. Strong communication and interpersonal skills required.

Other Requirements

Must pass TB test and background check (mandatory fingerprinting and screening by State of NM). Must have reliable transportation, valid NM driver's license, Agency insurable driving record and proof of current liability insurance. Knowledge of local resources available for coordination of programs and community outings is a plus. Must be able to lift 50 lbs. sporadically.
