Community Living Support Staff Albuquerque and Valencia County General Description of Duties Assist clients in their family homes or independent living environments in acquiring skills and/or completing activities of daily living. May include assistance with: banking, budgeting, paying bills, shopping, community safety skills, home management, cleaning, cooking and community recreation and leisure activity identification and participation. Will work flexible hours including evenings and weekends as necessary to meet client needs and schedules. Education and Training High School Diploma (or equivalent) required. Years of similar experience may be substituted for education in some instances. Must have the ability to speak English clearly, distinctly and cordially with internal and external customers of Adelante. Have the ability to read and write English in order to understand and interpret written procedures. This includes the ability to give and receive instructions in written and verbal forms and to effectively present information and respond to questions from clients, supervisors and co-workers. Must be able to handle behavior problems potentially involving verbal & physical aggression; individuals with medical problems. Have the ability to present a friendly, courteous image for the Agency to the general public, customers, and public officials. Experience working with mental retardation and developmental disabilities preferred. Must complete specific certifications (from our training department) and maintain those certifications throughout employment. Other Requirements Must have related experience and be able to work independently, without a great deal of supervision. Candidates will use their own vehicle, so you must have reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance. Must pass TB test and background check (Mandatory finger printing and screening by State of NM and FBI). Must have effective verbal communication and writing skills. Must be able to lift 50 lbs repeatedly throughout the shift as well as support 70-100 lbs. occasionally. Adelante is a drug-free workplace.
General Description of Duties Will be responsible for ensuring assigned health care services are provided/met in a timely manner. Coordinates appointments and arranges for transportation for client health services and ensures follow through; accompanies clients to appointments as required. May accompany clients who require emergency or urgent care to appropriate providers; ensures all follow-up directives are relayed to all relevant parties; performs all related administrative duties Will perform Community Living Support Staff duties as required. Administers medication as directed. Reports any changes in health or nutritional status; records accurate data into logs, case records, etc; assesses clients daily when in the home; performs 6 month health assessments on all clients. Interacts positively and constructively with all parties including but not limited to: Guardians, Case Managers, Therapists, Health Care professionals and all agency personnel; attends all trainings and other related meetings as required. Performs other duties as assigned. Education and Training RN or certification as a practical nurse (LPN) required. Five (5) years experience in health care with persons with developmental disabilities preferred. Three (3) years supervised nursing experience required. Licensed to practice nursing within the State of New Mexico. Must maintain CPR, First Aid and other trainings/certifications as required. Must have the ability to effectively communicate in English, with internal and external customers of Adelante. Have the ability to read and write English in order to understand and interpret written procedures. This includes the ability to give and receive instructions in written and verbal forms and to effectively present information and respond to questions from clients, supervisors and co-workers. Must be able to sit, stand, walk, bend, stoop, carry, and perform keyboarding/typing tasks frequently. Must be able to squat, reach above shoulder(s), kneel, balance, push, pull and lift 50 lbs. occasionally. Must be sufficiently mobile to visit various sites without prior notice. May include assistance with eating and all aspects of personal care . May include working with individuals with severe behavior problems. Will accompany residents on community outings. Must be able to document individual's progress and make recommendations for program changes. Education and Training High School Diploma (or equivalent) preferred with post high school, college preferred. Must have the ability to speak English clearly, distinctly and cordially with internal and external customers of Adelante. This includes the ability to give and receive instructions in written and verbal forms and to effectively present information and respond to questions from clients, supervisors and co-workers. General Description of Duties Responsible for completing, developing, reviewing, and disseminating client documentation. Provides direct client support through working with clients, providing crises management, implementing and monitoring therapy programs, and providing staff training. Completion and monitoring of client Individual Support Plans (ISP’s) required. Assure quality of client programming. Ensures that assigned client programs meet or exceed internal and external quality assurance standards. Facilitates client meetings and staff in – services as needed. Monitor funding of assigned clients. Complete and provide funding justification. Works closely with program managers, assistants, therapist, behavioral trainers and other agency staff to coordinate, implement and maximize programs/services for assigned clients. Education and Training High School Diploma (or equivalent) required with two years completion of a university program in education, psychology, rehabilitation or other related field preferred. Experience working with people with disabilities required. Extensive knowledge of therapies (PT, OT, SLP, etc.) and skills training principles preferred. Must have effective written and verbal communication skills. Must have the ability to effectively communicate in English, with internal and external customers of Adelante. Have the ability to read and write English in order to understand and interpret written procedures. This includes the ability to give and receive instructions in written and verbal forms and to effectively present information and respond to questions from clients, supervisors and co-workers. Other Requirements Must be pass background/fingerprint investigation and pre-employment drug screening. Must possess a valid NM driver's license and proof of auto liability insurance. Must have own reliable transportation and agency insurable driving record. Must have computer experience. Must be able to push/pull and lift 20-75 lbs., frequently. Adelante is a drug free workplace. General Description of Duties Responsible for daily operation of thrift store including customer relations, answering phones, tagging, stocking and selling merchandise. May set up displays of merchandise, assist with coordinating sales events and maintain general appearance of the retail area. Will process goods and donations. May provide back up to meet client's needs as related to placement, training and general supervision. Education and Training High School Diploma (or equivalent) required with post high school training in business preferred. Requires one year of general work experience with one year of retail experience preferred. Bilingual helpful. Must have the ability to effectively communicate in English, with internal and external customers of Adelante. Have the ability to read and write English in order to understand and interpret written procedures. This includes the ability to give and receive instructions in written and verbal forms and to effectively present information and respond to questions from clients, supervisors and co-workers. Must complete specific certifications (from our training department) and maintain those certifications throughout employment. Experience working with MR/DD population a plus. Other Requirements Must pass TB test, background check and pre-employment drug screen. Must have reliable transportation, valid NM driver's license, and agency insurable driving record. Must be able to operate cash register, tagging equipment and a 10 key adding machine as well as answer telephones as needed. Must be able to bend/stoop, squat, reach above shoulders, crouch, kneel, push/pull, carry frequently and stand for extended periods of time. Must be able to lift 50lbs.
